Job summary

Everwind Hospice is seeking a PRN On-Call Hospice RN to provide after-hours nursing support to patients and families, ensuring timely response to urgent needs and coordination of care per the hospice plan.

The role includes on-call evenings, nights, weekends, and holidays; requires Wyoming RN license and 1–2 years hospice/home health or acute care experience, plus a valid driver’s license and independent clinical decision-making.

Qualifications

• Current, unencumbered RN license - Wyoming.
• 1–2 years nursing experience (hospice, home health, or acute care preferred).
• Valid driver’s license and reliable transportation.
• Ability to work independently and make clinical decisions.

Responsibilities

• Responds to patient/family calls during assigned on-call hours.
• Provides telephonic triage and clinical guidance.
• Determines need for and performs urgent visits.
• Assesses patient condition and changes in status.
• Implements interventions for pain and symptom control.
• Administers medications and treatments per hospice plan of care.
• Provides end-of-life care and support.
• Coordinates with physician or NP as needed.
• Completes timely, accurate documentation of all calls and visits.
• Educates on symptom management, medication use, and what to expect.
