Job summary

Liberty Hospice in Cumberland County is seeking an experienced RN for full-time on-call coverage (Mon–Thurs, 5p–8a). You will respond to after-hours inquiries, assess patients, administer medications, and educate families about end-of-life care.

Collaboration with the interdisciplinary team and documentation in the EMR are essential. The ideal candidate holds an unrestricted RN license, CPR certification, strong clinical assessment skills, and the ability to work independently in a

Qualifications

Current unrestricted RN licensure in state of practice.
Successful completion of a nursing education from an approved school.
1 year hospice nursing experience preferred.

Responsibilities

Respond promptly to after-hours calls from patients, families or caregivers.
Perform assessments and provide interventions to manage pain and symptoms.
Administer medications and treatments per plan of care.
Provide education on end-of-life care, symptom management and medications.
Maintain clear communication with patients, families, caregivers and hospice team.
Document interactions and outcomes in EMR.
Perform after-hours admissions as needed.
Develop care plans per diagnosis and assessment findings.
Collaborate with physicians, RN case managers, social workers and chaplains.
