Qualifications

• Current unrestricted RN licensure in the state of practice.
• Two years of clinical nursing experience and at least one year of hospice nursing preferred.
• CPR certification required.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and interpersonal skills.
• Ability to work independently and make sound decisions.
• Compassionate, patient-centered approach to care.
• Proficiency with electronic medical records and basic computer skills.
• Valid driver’s license.

Responsibilities

• Respond promptly to after-hours calls regarding symptom management or urgent concerns.
• Perform assessments and provide interventions to manage pain and symptoms.
• Administer medications and treatments per the plan of care.
• Educate patients and families on end-of-life care and medication usage.
• Maintain clear communication with patients, families, and the hospice team.
• Document interactions, assessments and outcomes in the EMR.
• Coordinate after-hours admissions as needed.
• Develop care plans with the team consults and physician guidance.
• Adhere to hospice policies, procedures, and regulatory requirements.
• Maintain confidentiality and HIPAA compliance.
