Job summary

Interim HealthCare Hospice in Fort Wayne, IN, is seeking Hospice Registered Nurses (RNs) for PRN work. Provide home-based care focused on quality of life for patients with six months or less to live. You will join an interdisciplinary team and educate families on prognosis, care plans, and hospice goals.

We offer 1:1 nurse-to-patient ratios, mileage reimbursement, flexible assignments, and online training with CEUs. Indiana RN license and CPR certification are required.

Qualifications

• Must have RN license in Indiana and CPR certification.
• Minimum of 1 year of nursing experience, ideally in hospice or home healthcare.
• Knowledge of state and federal home health regulations.
• Clinical proficiency in nursing, strong communication skills and compassion.

Responsibilities

• Provide home-based care to patients with six months or less to live, focused on quality of life.
• Work as part of an interdisciplinary team to carry out the patient’s plan of care.
• Perform patient assessments, check vitals, evaluate breathing, monitor pain, administer prescribed medication and treatments.
• Ensure accessibility of medical equipment, prescribed oxygen, medication and other supplies.
• Educate family on the patient’s prognosis, changes in care plan, medication and hospice goals.
• Take a holistic approach to care that engages the patient’s mind, body, spirit and family.
