Job summary

Three Oaks Hospice, Inc. seeks an experienced registered nurse to provide skilled nursing care in patients' homes across Lufkin and surrounding areas.

You will coordinate with the interdisciplinary team to deliver compassionate end-of-life care and ensure comfort and dignity for patients and families. The role requires a current RN license, CPR certification, and strong communication, interpersonal, and organizational skills.

Qualifications

• Current RN license and valid state driver’s license.
• Two years of RN experience in a clinical care setting; hospice/home health preferred.
• Current CPR certification.
• Proficient with computers; Microsoft Office experience preferred.

Responsibilities

• Initial and ongoing nursing assessments in the patient home setting.
• Coordinate care with the interdisciplinary team and plan of care.
• Educate patients and families on disease process, self-care, and end-of-life care.
• Provide training to other staff as needed.
• Prepare clinical and progress notes showing progress toward goals.
• Coordinate patient/family services and priorities with the IDT.
• Perform on-call nursing duties as assigned.
• Communicate changes in patient needs to physicians and team.
