Assurance Home Care in Sacramento, CA is seeking a compassionate Registered Nurse (RN) to join our home health team, serving Roseville, Lincoln and surrounding areas. You will assess patients, develop individualized care plans, and coordinate care with physicians.

Qualifications

• Current California Registered Nurse (RN) license.
• Current BLS certification.
• Valid driver’s license, reliable transportation, and auto insurance.
• Excellent assessment, communication, and critical thinking skills.
• Ability to work independently while collaborating with an interdisciplinary team.

Responsibilities

• Provide skilled nursing care to patients in their homes and develop individualized care plans.
• Perform comprehensive assessments, administer medications and treatments, and educate patients and caregivers.
• Coordinate care with physicians and the interdisciplinary team to promote safe recovery.
• Support patients in managing chronic conditions and reducing hospitalizations.
