Interim HealthCare of Western Wisconsin is seeking a Hospice Registered Nurse (RN) to provide compassionate, home-based care in Hudson, WI and surrounding areas. You will assess patients with six months or less to live, work with an interdisciplinary team, and support families through prognosis and care planning.

The role offers competitive pay ($80,000 - $95,000 per year), mileage reimbursement, on-call pay, and opportunities for bonuses, along with ongoing training and benefits.

Qualifikationen

• Graduate of an accredited nursing program and active RN license in Wisconsin.
• Minimum of 1 year of nursing experience, ideally in hospice or home health.
• CPR certification.
• Knowledge of state and federal home health regulations.
• Clinical proficiency, strong communication skills and compassion.

Aufgaben

• Provide home-based care to patients with six months or less to live, focused on quality of life.
• Work as part of an interdisciplinary team to carry out the patient’s plan of care.
• Perform patient assessments, check vitals, evaluate breathing, monitor pain, administer prescribed medications and treatments.
• Ensure accessibility of medical equipment, prescribed oxygen, medications and other supplies.
• Educate family on patient prognosis, changes in care plan and hospice goals.
• Take a holistic approach to care that engages mind, body, spirit and family.
