Schedule is Mon-Fri with mileage reimbursement and a pay range based on experience. Responsibilities include patient-centered nursing care, care planning with families and physicians, comprehensive assessments, and ongoing education of patients and caregivers.

Qualifications

• RN license in the state of practice.
• At least one year of nursing experience.
• Reliable transportation and valid driver's license.

Responsibilities

• Provide individualized nursing care that maximizes comfort and quality of life.
• Partner with patients, families, physicians, and the interdisciplinary group to create and carry out care plans.
• Perform comprehensive assessments-physical, emotional, social, and spiritual to guide care decisions.
• Educate and support patients and caregivers, meeting them where they are.
• Coordinate care across disciplines to ensure continuity and dignity.
• Document care clearly and accurately to reflect patient needs and progress.
• Participate in on-call or admission rotations as needed.
• Engage in ongoing learning and professional development.
