Job summary

All Advanced Home Care Inc. seeks an experienced Registered Nurse to join our in-home care team in Florida. You will assess patients, develop and adjust personalized care plans, and collaborate with a multidisciplinary group to deliver skilled nursing services in home settings.

Qualifications

• Valid and unrestricted Florida RN or LPN license.
• RN - Bachelor's or Associate's Degree in Nursing.
• 1–2 years of nursing experience; home health care experience preferred.
• Current BLS certification.
• Licensed driver with insured vehicle per organization requirements.
• Excellent observation, verbal and written communication, problem-solving and basic math skills.
• Ability to work independently and adapt to varying schedules.
• Knowledge of health and safety guidelines and willingness to follow Kinnser EMR system.

Responsibilities

• Conduct thorough assessments of patients and families, using physical exams and medical history to develop a care plan.
• Perform in-home visits and provide skilled nursing care, adjusting plans as needed.
• Educate patients and families on disease prevention, treatment options, and wellness.
• Administer physician-prescribed treatments and communicate patient progress to physician and Clinical Manager.
• Document observations and treatments according to agency protocols and maintain HIPAA compliance.
• Coordinate discharge planning as part of the plan of care.
• Maintain accurate electronic health records and ensure confidentiality.
• Participate in conferences, staff meetings, and performance improvement activities.
