Job summary

Oakbrook Hospice and Palliative is seeking a Hospice Registered Nurse in Naperville, IL. The RN administers nursing care to patients in their homes and coordinates with the interdisciplinary team, families, and referring agencies.

Responsibilities include comprehensive assessments, care planning, patient education, and on-call coverage. Travel locally is required, with flexible scheduling and potential exposure to infectious diseases in home settings.

Qualifications

• Education: Graduate from an accredited Diploma, Associate, or Baccalaureate School of Nursing.
• Licensure: Current state RN license and valid driver's license.
• Experience: Experience as an RN in a clinical care setting; hospice experience preferred.
• Skills: Nursing skills per standards; good interpersonal skills; must read/write English.
• Transportation: Reliable transportation and valid auto liability insurance.

Responsibilities

• Initial and ongoing comprehensive assessments of patient needs and eligibility for hospice care.
• Develops and documents the plan of care to ensure quality and continuity.
• Educates patients and families on disease process, self-care, end-of-life care, nutrition.
• Coordinates with Interdisciplinary Team; communicates changes and coordinates care.
• Provides on-call coverage and on-site care in patients' homes; maintains documentation.
