Qualifications

• Licensed driver with automobile insured in accordance with state requirements.
• Current CPR certification.

Responsibilities

• Performs comprehensive assessments of client status.
• Reports changes in client status to physician and team members.

Skills

Strong communication and interpersonal skills

Education

Graduate of an accredited school of professional nursing. BSN, PHN preferred
