You will conduct assessments, develop care plans, administer medications, and collaborate with physicians, social workers, and caregivers to ease patient and family needs.

Qualifications

• RN licensure required.
• Hospice/palliative experience preferred.
• Strong assessment, communication, and interpersonal skills.
• Ability to collaborate in a multidisciplinary team.

Responsabilités

• Provide compassionate, end-of-life care in home and facility settings.
• Conduct comprehensive patient assessments and develop individualized care plans.
• Administer medications and manage complex symptom and pain control.
• Coordinate with physicians, social workers, and team to support patients and families.
• Document in EMR and educate caregivers on care processes and techniques.
• Participate in on-call rotations and quality improvement initiatives.
