Compassus is seeking a Hospice Registered Nurse / RN Case Manager to join our compassionate team in the Woodlands, Texas. You will assess patients at home, develop individualized care plans, and coordinate with the care team to support families with dignity and comfort.

Qualifications

• RN license in the state you work.
• Two years of nursing experience, hospice experience preferred.
• Current CPR and BCLS certification.
• Valid driver's license required.

Responsibilities

• Provide patient assessments and care planning.
• Coordinate with the care team to deliver comprehensive hospice services.
• Educate families and caregivers on care plans and home safety.
• Manage symptoms and comfort to enhance quality of life.
• Document patient progress and maintain compliance with regulations.
