Job summary

Heart’n Soul Hospice is seeking a Registered Nurse Case Manager to join our Fort Myers team. The role includes comprehensive patient assessments, developing and adjusting the care plan, and coordinating with physicians, IDG members, and caregivers to ensure high-quality end-of-life care.

Requirements include a nursing degree, an active RN license, and 2 years of RN experience. Benefits include medical, dental, vision, PTO, and a company car for qualifying individuals, with mileage reimbursement.

Qualifications

• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
• Current State Registered Nurse License.
• Two years of RN experience preferred in healthcare/clinical setting.

Responsibilities

• Initial and ongoing comprehensive assessments of patient needs and ADLs.
• Implement individualized care plans and revise as needed.
• Collaborate with care teams and IDG to coordinate patient care.
