Job summary

UnitedHealth Group seeks a Per Diem Registered Nurse Case Manager to deliver the Plan of Care for hospice patients. You will evaluate and adjust care to maintain quality, adhering to hospice policies and objectives.

Responsibilities include initial eligibility evaluations within 48 hours of referral, ongoing reassessment, care coordination, and thorough documentation. Collaboration with families, physicians, and the IDG is essential.

Qualifications

• Current unrestricted RN licensure in state of practice.
• Minimum 1 year of clinical nursing experience.
• Valid driver's license, reliable vehicle, or access to public transportation.
• LA state requirement: two years RN experience; hospice RN experience may substitute for RN experience.

Responsibilities

• Make the initial nursing evaluation to determine eligibility for hospice services within 48 hours of referral.
• Assess and reassess patient needs every 15 days or as needed.
• Initiate and coordinate the plan of care with accurate clinical notes.
• Document problems, goals, interventions, and patient/family response to hospice care.
• Collaborate with patient/family, attending physician, and IDG to provide daily care.
