Wesley Homes is seeking a part-time RN Case Manager for Hospice in Washington. You will coordinate comprehensive hospice care, lead the interdisciplinary team, and ensure patient comfort and dignity in end-of-life care. The role emphasizes collaboration with physicians, social workers, and family members.

Requires WA RN license, CPR, and hospice experience. On-site, day-shift, with strong emphasis on compassionate, patient-centered care and compliance with regulatory standards.

Qualifications

• Graduate of an accredited nursing program.
• Active RN license in Washington State or eligibility to practice.
• Current CPR certification.
• Minimum of one year nursing experience.
• Knowledge of hospice and palliative care principles.

Responsabilités

• Conduct comprehensive initial and ongoing assessments of patients with terminal illnesses.
• Develop, implement, and revise hospice plans of care.
• Coordinate physician orders and communicate with the inter-disciplinary group.
• Provide skilled nursing services including symptom management and care coordination.
• Educate patients and families regarding disease progression, hospice services, and end-of-life expectations.
• Maintain documentation and compliance with HIPAA and hospice requirements.
