Job summary

The Pennant Group seeks an experienced Hospice Registered Nurse Case Manager (RN) to join Puget Sound Home Health and Hospice in South King County, WA. This full-time, on-site role focuses on patient assessments, care planning, symptom management, and family education within a hospice setting.

You will participate in IDT meetings, coordinate with a care team, and provide compassionate end-of-life support, with a strong emphasis on quality patient care and regulatory compliance.

Qualifications

• Current RN licensure to practice in Washington state.
• BLS certification from AHA is required.
• Hospice experience strongly preferred.
• Must have a valid driver’s license and reliable vehicle.

Responsibilities

• Perform patient assessments (initial and ongoing).
• Develop and update individualized care plans.
• Administer medications and manage symptoms.
• Educate and support patients and families.
• Attend and participate in IDT meetings.
• Conduct home visits as needed and document in EMR.
• Communicate with physicians and care team; respond to urgent calls.
