Alliance for Care at Home is seeking a Hospice Registered Nurse (RN) to join our interdisciplinary team in a compassionate, on-site role. You will provide care with autonomy and support within a mission‑driven organization that values well‑being and professional growth.

You’ll coordinate with patients, families, and physicians to develop and carry out care plans, perform thorough assessments, educate caregivers, and participate in on‑call rotations while upholding dignity and comfort for hospice

Qualifikationen

• RN license in the state of practice.
• At least one year of nursing experience.
• Reliable transportation, valid driver’s license, and appropriate auto insurance.

Aufgaben

• Provide individualized nursing care that maximizes comfort and quality of life.
• Partner with patients, families, physicians, and the interdisciplinary group to create and carry out care plans.
• Perform comprehensive assessments—physical, emotional, social, and spiritual—to guide care decisions.
• Educate and support patients and caregivers, meeting them where they are.
• Coordinate care across disciplines to ensure continuity and dignity.
• Document care clearly and accurately to reflect patient needs and progress.
• Participate in on‑call or admission rotations as needed.
• Engage in ongoing learning and professional development.
