Job summary

Bluegrass Care Navigators in Kentucky is hiring a Hospice RN Case Manager to deliver compassionate, comprehensive care to patients in home settings. The role requires strong listening, autonomy, and teamwork within an interdisciplinary group.

You will develop holistic care plans aligned with patient goals, monitor symptoms, and provide support to patients and families during the hospice journey. Evening/on-call shifts rotate; schedule is Monday through Friday with weekend rotation.

Qualifications

• Graduate of an accredited school of registered nursing.
• Licensed as a Registered Nurse by the Kentucky Board of Nursing or a participating Compact State Board of Nursing.
• Minimum of one year general nursing experience preferred; new graduate nurses will be considered.
• Demonstrates strong clinical nursing skills with professionalism during emotionally challenging situations.
• Provides compassionate, patient-centered care while serving diverse populations with respect and cultural sensitivity.
• Adheres to accepted professional, ethical, and legal standards of nursing practice.
• Demonstrates understanding of and commitment to hospice philosophy and the principles of death and dying.
• Maintains current CPR certification.

Responsibilities

• Deliver compassionate, comprehensive hospice care to patients in home care settings.
• Develop and implement holistic care plans based on the patient’s goals and preferences.
• Advocate for patients, families, and caregivers, coordinating care within an interdisciplinary team.
• Monitor symptoms and follow up to evaluate treatment efficacy and adjust plans as needed.
• Provide emotional support to patients and caregivers throughout the hospice journey.
• Communicate effectively within an interdisciplinary team and uphold professional standards.
