Job summary

Care Initiatives Hospice - Albia is seeking an experienced RN Case Manager to deliver compassionate, in-home patient care across multiple settings including home, long-term care, assisted living, and hospital environments. You will coordinate comprehensive plans of care with physicians and an interdisciplinary team to ensure optimal patient outcomes.

Qualifications

• Iowa RN license required.
• CPR certification required.
• Hospice or palliative care experience preferred.
• Two years of verifiable RN/LPN experience required.
• Valid driver’s license, automobile liability insurance, and ability to travel regionally.

Responsibilities

• Deliver high-quality patient care in the patient’s residence using nursing knowledge and skills.
• Develop and oversee care plans under physician direction with interdisciplinary involvement.
• Perform initial and comprehensive patient assessments per policies and procedures.
• Conduct ongoing visits and interdisciplinary patient assessments.
• Provide accurate clinical documentation and educate patients, caregivers, and families.
