Phoenix Home Care & Hospice in North Kansas City, MO seeks a Registered Nurse Case Manager for Hospice home health. You will provide compassionate, patient-centered care, coordinate with the interdisciplinary team, and guide families through goals of care and treatment decisions.

Ideal candidate has an active Kansas or compact RN license, hospice experience preferred, valid driver’s license, and willingness to train.

Qualifications

• Active Kansas or Compact RN license.
• Hospice experience preferred; training provided.
• Valid driver’s license and auto insurance.

Responsibilities

• Provide compassionate, hands-on hospice care.
• Support and educate patients and families.
• Complete assessments and ongoing evaluations.
• Collaborate with physicians and the hospice team.
• Address patients’ physical, emotional, and comfort needs.
