Job summary

Phoenix Home Care & Hospice is seeking a Registered Nurse (RN) for Home Health in Lebanon, Missouri. You will deliver hands‑on patient care in patients' homes and coordinate services with doctors, insurers, and visiting nurses to craft personalized care plans.

The role requires strong assessment and communication skills, collaboration with therapy teams, and supervision of LPN visits. An active Missouri RN license is required, with Home Health experience preferred.

Qualifications

• Active Missouri or compact RN license.
• Driver's license is required.
• Home Health or Critical Care experience preferred.

Responsibilities

• Manage caseload including supervising visits by LPNs.
• Deliver hands‑on home health care and coordinate patient care.
• Educate patients on health conditions and medications.
• Collaborate with physicians to obtain orders and create care plans.
• Coordinate with PT/OT/Speech therapy teams.
