Job summary

Advocate Health is seeking a Hospice Nurse Case Manager (RN) to provide skilled nursing care and coordinate comprehensive home hospice services in Floyd, GA and surrounding counties. You will conduct assessments, develop care plans, and support patients and families through the hospice journey.

This role requires an RN license in Georgia, reliable transportation, and at least two years of nursing experience with end-of-life care.

Qualifications

• Graduate of an accredited Registered Nursing program.
• Current Georgia RN license and valid Georgia driver's license; reliable transportation.
• Minimum of two years of nursing experience; end-of-life care experience preferred.

Responsibilities

• Conduct admission, pre-admission, routine and on-call visits.
• Perform comprehensive physical, psychosocial and environmental assessments.
• Develop, implement and revise individualized care plans.
• Provide skilled nursing care and symptom management in home hospice.
• Coordinate with interdisciplinary team and document care operations.
