Job summary

Capstone Hospice in Georgia is seeking an experienced RN Case Manager to deliver compassionate, patient-centered end-of-life care. You will conduct assessments, develop care plans, manage pain and symptoms, and educate families on the hospice process, guiding patients and their loved ones through the final journey.

This full-time role requires graduation from an accredited nursing program and current Georgia RN licensure, with a minimum of two years of RN experience and active patient contact.

Qualifications

• Graduate from an accredited nursing program; RN licensed in Georgia.
• Minimum of 2 years as an RN with active patient contact preferred.
• Ability to work independently with compassionate care.

Responsibilities

• Observe and assess patients; develop and update care plans with IDG involvement.
• Manage pain and symptoms; educate families on hospice processes.
• Document visits in the EMR and coordinate with the IDG and team.
• Attend team conferences and manage a caseload of 10–14 patients.
