Job summary

Holisticare Hospice is seeking a full-time Hospice RN Case Manager to join our nursing team providing end-of-life care in the Delaware County area. The role involves in-home visits, coordination of services, and guiding patients and families through the hospice journey.

Responsibilities include delivering hands-on nursing care, collaborating with the Clinical Director, and educating families to create patient-centered care plans. Excellent compassion and clinical excellence are essential.

Qualifications

• Hold current unencumbered license as a Registered Nurse.
• Minimum one year experience as an RN in a medical surgical/acute care setting; hospice experience a plus.
• Must be computer proficient in typing and various programs, including EMR.
• Possess and maintain CPR certification if required by state.
• Current driver's license, vehicle insurance, and access to dependable transportation.

Responsibilities

• Deliver hands-on hospice nursing care and coordinate services.
• Guide patients and families through every step of their hospice journey.
• Collaborate with the Clinical Director to assess eligibility and suitability of clients for hospice services.
• Provide comfort and quality of life through skilled nursing care.
• Educate and advocate for patients and families to create personalized care plans.
