Central Coast VNA and Hospice is seeking a Hospice RN Case Manager in Salinas, CA. This full-time position offers an hourly salary range of $60.92 to $66.08, reflecting experience and location.

The Hospice RN Case Manager collaborates with physicians and the interdisciplinary team to coordinate home-based care, assess patient needs, and promote comfort and quality of life for patients and families. Strong nursing skills in home health and hospice are essential.

Formación

• Active California RN license.
• Minimum of one year home health or hospice nursing experience.
• Knowledge of home health, hospice, and palliative care.
• Current CPR certification.
• Ability to work with physicians and interdisciplinary team members.

Responsabilidades

• Collaborate with physicians and the care team to coordinate home care needs.
• Provide skilled nursing care focused on comfort and quality of life.
• Refer and coordinate other disciplines and transitions to other levels of care as needed.
• Educate patients, families, and caregivers; prepare for discharge when appropriate.
