Job summary

Bristol Hospice in Visalia, CA is hiring an RN Case Manager to provide compassionate, whole-person care and advocacy for patients and families.

This full-time, day-shift role offers $50.00–$52.00 hourly and a comprehensive benefits package, with PTO, health benefits, tuition reimbursement, and retirement plans.

Qualifications

• A valid Registered Nurse license in good standing in the state you will work in.
• CPR certification (current) is required.
• A valid driver’s license and reliable transportation.

Responsibilities

• Make a true difference—one patient, one family, one compassionate moment at a time.
• Develop and implement individualized care plans based on current and prior medical history
• Deliver attentive, one-on-one support to patients and their families, ensuring comfort, dignity, and understanding
• Conduct thorough assessments of physical, emotional, and spiritual needs throughout the care journey
• Accurately monitor and document vital signs including temperature, pulse, respiration, and blood pressure
• Administer physician-ordered medications and treatments with skill and empathy
• Educate patients and their families on care techniques and symptom management
• Track and communicate changes in patient condition, ensuring timely and effective responses
• Keep detailed and up-to-date clinical notes to reflect patient progress and quality of care provided
• Collaborate with interdisciplinary teams to ensure comprehensive care delivery
• And contribute to other supportive duties as part of a compassionate, mission-driven team
• And additional duties as assigned
