Job summary

Bristol Hospice in Grass Valley, CA is seeking dedicated RN Case Managers who are passionate about walking with patients and families through every step of end-of-life care. You will assess needs, develop care plans, monitor vitals, administer physician-ordered treatments, educate families, and collaborate with interdisciplinary teams to deliver comfort with dignity.

Qualifications

• Must hold a valid RN license in the state you will work in.
• CPR certification (current).
• Reliable transportation and a valid driver’s license.

Responsibilities

• Develop and implement individualized care plans based on patient history and needs.
• Deliver one-on-one, compassionate support to patients and families.
• Monitor vital signs and document accurately.
• Administer physician-ordered medications and treatments with empathy.
• Educate patients and families on care techniques and symptom management.
