Covenant Living is seeking a dedicated Hospice RN Case Manager to plan, organize and direct hospice care services in the Holmstad Batavia area and surrounding communities. The role emphasizes community health education and collaborating with families to meet patient needs.

The position is full-time and requires strong nursing skills, effective communication with residents and healthcare professionals, and experience in hospice care.

Qualifications

• Associate’s Degree in Nursing required.
• Bachelor’s Degree in Nursing preferred.
• RN license and CPR certification preferred as applicable by state.
• 1+ year hospice care experience preferred.

Responsibilities

• Complete initial patient and family assessment to determine hospice needs.
• Provide comprehensive physical history and ongoing reassessment of patient status.
• Develop and revise the care plan with goals based on nursing diagnosis.
• Administer medications and treatments per physician orders.
• Counsel patients and families and coordinate with the Interdisciplinary Team (IDT).
