Job summary

Covenant Living is seeking a Hospice RN Case Manager to plan, organize, and direct hospice care for residents in the Holmstad area (Batavia) and surrounding locations. The role emphasizes home and community-based nursing with an interdisciplinary team approach.

The position requires an Associate’s Degree in Nursing, an active RN license, CPR as required, and 1+ year hospice experience is preferred. Full-time with comprehensive benefits is offered.

Qualifications

• Requires Nursing degree and RN licensure with CPR as applicable.
• Hospice case management experience preferred (1+ year).
• Ability to communicate with residents, families, and healthcare professionals.

Responsibilities

• Complete initial patient and family assessments for hospice care needs.
• Perform physical assessments and medical history reviews.
• Regularly re-evaluate nursing needs and adjust care plans.
• Lead and participate in IDT meetings and case conferences.
• Administer prescribed medications and provide family counseling.
