Job summary

Covenant Living in Batavia, IL is seeking an experienced Hospice RN Case Manager for full-time, supporting Covenant Living Holmstad and nearby locations. The role emphasizes assessment, care planning, and collaboration with the Interdisciplinary Team to meet patient and family needs at home in the community.

The candidate should hold an Associate’s Degree in Nursing with RN licensure; Bachelor’s is preferred.

Qualifications

• Required Degree: Associate's Degree, Nursing.
• Preferred Degree: Bachelor’s Degree, Nursing.
• Preferred Certificate(s): RN License and CPR, if required by state.
• Experience: 1+ Years of hospice care experience preferred.

Responsibilities

• Completes an initial assessment of patient and family to determine hospice care needs.
• Develops and revises the plan of care with nursing diagnoses and goals.
• Administers medications and treatments as prescribed; counsels patient and family.
• Participates in IDT meetings and case conferences; communicates updates as needed.
