Covenant Living and CovenantCare at Home are seeking a Hospice RN Case Manager to plan, organize, and direct hospice care services within home health settings. The role emphasizes community health education, patient/family assessment, and collaboration with the interdisciplinary team.

Ideal candidates hold an RN license with CPR, 1+ year hospice experience, and strong communication skills to address diverse patient needs and ensure quality end-of-life care in residents' homes.

Qualifications

• Associate degree in nursing required. Bachelor’s degree in nursing preferred.
• RN license and CPR may be required by state. 1+ year hospice care experience preferred.
• Ability to communicate effectively with residents and healthcare professionals; patient confidentiality and privacy knowledge.

Responsibilities

• Plans, organizes and directs hospice care services.
• Completes initial patient and family assessment to determine hospice needs.
• Develops and revises care plans with interdisciplinary team; conducts IDT meetings.
