Job summary

Hope Hospice is seeking a Hospice Nurse Case Manager to assess patient and family needs, plan and provide skilled nursing care under the physician's plan, and coordinate services across the care team in Contra Costa and Alameda counties.

The role is full-time with a Monday–Friday schedule, offering competitive pay and benefits. Candidates must hold a California RN license and may need CPR certification and driving requirements.

Qualifications

• One year experience as a professional nurse in the past three years or Bachelor’s degree in Nursing from an accredited School of Nursing.
• Experience in acute care/home health and/or hospice setting preferred.

Responsibilities

• Identify patient/family needs and provide skilled nursing assessment, planning and supportive care.
• Coordinate care with attending physicians and hospice staff.
• Maintain caseload productivity per agency protocol.
