Job summary

Emblem Hospice in the Phoenix area is seeking a dedicated Hospice RN Case Manager to support patients in homes and communities across Scottsdale and Paradise Valley. You will assess patients and families, develop individualized care plans, administer care, and guide families through difficult decisions with compassion and professionalism.

The role emphasizes collaboration with the interdisciplinary team, ongoing education for patients and families, and accurate documentation.

Qualifications

• Must be licensed as an RN in Arizona.
• Hospice/home health experience preferred.
• Strong clinical assessment and care-planning skills.

Responsibilities

• Complete comprehensive patient and family assessments.
• Develop and implement individualized care plans and update as needed.
• Provide skilled nursing care, administer medications and manage symptoms.
• Educate patients and families on care techniques, medications and comfort measures.
• Maintain accurate clinical documentation and communicate changes to physicians.
• Participate in interdisciplinary team meetings and quality improvement initiatives.
• Provide emotional support to patients and families; supervise hospice aides as needed.
