Job summary

Emblem Hospice in the Central Phoenix area is hiring a full-time Hospice RN Case Manager. You will deliver home-based nursing care, guide families through care decisions, and provide comfort and support during life’s moments. The role emphasizes collaboration with an interdisciplinary team and ongoing professional development.

Requires an active Arizona RN license and at least 2 years in nursing, with hospice or home health experience preferred. Driving and reliable vehicle are expected.

Qualifications

• Current RN license in the State of Arizona.
• Minimum of 2 years nursing experience; hospice or home health preferred.
• Experience with Homecare Homebase (HCHB) is preferred but not required.

Responsibilities

• Complete comprehensive assessments of patients and families to determine hospice needs.
• Develop and implement individualized care plans.
• Provide skilled nursing care and manage symptoms.
• Educate patients and families on care techniques and medications.
• Maintain accurate clinical documentation and communicate changes to physicians.
• Participate in interdisciplinary team meetings and quality improvement initiatives.
• Provide emotional support to patients and families.
• Supervise hospice aides and delegate tasks appropriately.
