St. John’s United Hospice is seeking a Registered Nurse Case Manager to provide compassionate hospice care in patients' homes and community settings.

You will work under the Hospice Clinical Team Leader and partner with an interdisciplinary team to deliver a comprehensive plan of care addressing physical, emotional, and spiritual comfort at end of life. The role emphasizes independence, flexible scheduling, and maintaining a strong patient-to-RN ratio, with opportunities for ongoing hospice

Qualifications

• RN license in Montana required; BSN preferred.
• Minimum 2 years hospice/home health/SNF nursing experience.
• CPR certification required.
• Strong communication and computer skills.

Responsabilités

• Provide hospice nursing care and management in patients' homes, assisted living, or long-term care.
• Assess and maintain hospice services to meet client needs throughout the continuum of care.
• Maintain accurate client records using computerized documentation and agency protocols.
• Collaborate with Home Health Aides, LPNs, social workers, chaplains, and medical directors; supervise staff as needed.
• Perform routine home visits and occasional after-hours on-call duties; support Hospice philosophy of care.
