Job summary

JourneyCare is seeking a compassionate Hospice Registered Nurse (RN) Case Manager to provide patient-centered, end-of-life care in the Deerfield area, encompassing Lake Forest, Waukegan, and Zion, IL.

The ideal candidate will manage a dedicated caseload while delivering skilled nursing care with a focus on comfort and dignity. This full-time position offers a competitive salary with benefits, including medical, dental, and vision insurance, along with generous PTO.

Qualifications

• Current CPR and First Aid certification required.
• Valid driver's license and proof of insurance needed.

Responsibilities

• Manage a dedicated hospice caseload as the primary RN Case Manager.
• Perform patient assessments, admissions, and individualized care planning.
• Deliver skilled nursing care focused on comfort and dignity.
• Coordinate care with physicians and interdisciplinary team members.
• Conduct routine visits and adjust care plans as needed.
• Provide end-of-life support, including family education and pronouncement.
• Supervise Home Health Aides in compliance with regulations.
