Job summary

JourneyCare Hospice is seeking a Hospice RN to join our team in Illinois. The role offers new competitive salaries, immediate opening, generous time off, and a 401K with match.

You will manage patient and family caseloads through assessment, admission, and ongoing care planning, coordinating with a multidisciplinary team to ensure high-quality hospice care. The position requires an RN license, CPR/First Aid, and a driver’s license.

Qualifications

• Graduate from an accredited registered nursing program.
• Licensed as a registered nurse in the state of practice.
• Current CPR and First Aid certifications.
• Knowledge of quality assessment and assurance procedures.
• Valid driver’s license and proof of insurance.

Responsibilities

• Assume primary responsibility for patient and family caseload, including assessment, eligibility, admission processing and care planning.
• Obtain data from patient and family across physical, psychological, social, and spiritual factors and coordinate with the team.
• Communicate with attending physician and interdisciplinary team to coordinate care.
• Provide regular visits to reassess patient needs and update the care plan.
• Perform clinical pronouncement of death and support the family.
• Maintain accurate patient records, charts, and timely care plans.
• Supervise and document delivery of care by Home Health Aide per guidelines.
