Job summary

Stillwater Hospice in Rapid City, SD is seeking an experienced Hospice RN Case Manager to join a growing team. You will coordinate patient care, perform comprehensive assessments and collaborate with the interdisciplinary group to ensure high-quality end-of-life care for patients in home settings.

The role requires an active registered nurse license in the state, with one to two years of hospice or home care experience preferred.

Qualifications

• Graduate of an accredited school of nursing.
• Current RN licensure in the state.
• Bachelor’s degree preferred with hospice care experience.

Responsibilities

• Completes an initial, comprehensive assessment of patient and family to determine hospice needs.
• Provides professional nursing care by utilizing the nursing process per state Nurse Practice Act.
• Develops a care plan with goals based on nursing diagnosis and palliative actions.
• Initiates and revises the plan of care as patient status changes.
• Acts as Case Manager coordinating patient care for assigned caseload.
• On-call duties as defined by policy.
