Alpha Omega Hospice is seeking a Full Time Hospice Registered Nurse Case Manager to join our caring nursing team serving Johnson County. You will deliver hands-on hospice nursing care, coordinate services, and guide patients and families through their hospice journey.

Qualifications

• Hold current unencumbered Registered Nurse license.
• Minimum 1 year RN experience in medical-surgical/acute care; hospice a plus.
• Proficient with computers and EMR systems.
• CPR certification if required by state.
• Valid driver’s license and transportation.

Responsibilities

• Empower Through Engagement: respond to clinical referral information with care and expertise.
• Collaborative Care Champion: work with Clinical Director to assess eligibility for services.
• Nurturing Direct Care: deliver hands-on hospice nursing care and coordinate services.
• Nursing with Heart: provide top-notch nursing care rooted in proven principles.
• Educator and Advocate: educate patients and families to create personalized care plans.
• Call of Compassion: participate in call duties to be there when it matters most.
