Job summary

Chapters Health System in Florida is seeking an RN Case Manager to assess patient and family needs, coordinate the Plan of Care with the Interdisciplinary Team (IDT), and provide clinical, palliative and supportive care to the patient/family while supporting home-based care.

The role requires an active RN license in Florida, at least 1 year of hospice or hospital experience, EMR/EHR experience, and a valid driver’s license with reliable transportation.

Qualifications

Current RN license in the state of employment.
Minimum 1 year nursing experience in hospice or hospital setting.
Alzheimer's Disease and Related Dementias Training through Florida Department of Elder Affairs (PACE employees).
Experience with EMR/EHR systems.
Valid driver’s license and auto insurance per policy.

Responsibilities

Provides and manages direct care to patients and families as part of the Interdisciplinary Team.
Coordinates the Plan of Care reflecting patient needs and evolving conditions.
Educates patients, families, and caregivers about disease process, palliation, and safety.
Initiates skilled nursing interventions to alleviate symptoms and maximize comfort.
Communicates with physicians and IDT with SBAR and timely documentation.
