Job summary

Hospice of the North Coast is seeking an experienced registered nurse to join our hospice team as an RN Case Manager. You will initiate and coordinate care plans, provide skilled and palliative care, and collaborate with physicians and the interdisciplinary team to support patients and families at end of life.

This full-time role runs Monday to Friday 8:30am – 5:00pm. Requirements include CA RN license, BSN preferred, hospice certification desired, and a minimum of one year of professional

Qualifications

• Graduate of an accredited school of nursing.
• Current CA RN License. BSN preferred. Certification in Hospice and Palliative Nursing desired.
• Minimum of one-year experience as a professional nurse within the last 3 years or have a Baccalaureate in Nursing
• Valid driver's license, reliable transportation and auto insurance.

Responsibilities

• The RN initiates and coordinates the hospice care plan and provides skilled and palliative care.
• Coordinates services with the attending physician, Hospice Medical Director, and interdisciplinary team.
• Participates in patient care conferences and supervises Home Health Aides.
• Reviews and updates the plan of care and conducts prescribed medical treatments including pain management and symptom control.

Education
