Job summary

Sanctuary Hospice is seeking a dedicated Registered Nurse Case Manager (RNCM) to join our nursing team providing compassionate end-of-life care across the community. In this full-time role, you will deliver direct hospice nursing, coordinate services, and guide patients and families through every step of their journey.

Qualified candidates hold an active RN license, have at least 1 year of medical-surgical/acute care experience (hospice a plus), and are proficient with EMR systems.

Qualifications

• Hold current unencumbered RN license.
• Minimum one year RN experience in medical-surgical/acute care setting; hospice experience a plus.
• Proficient with computers and EMR; CPR certification may be required by state.
• Valid driver's license, vehicle insurance, and reliable transportation.

Responsibilities

• Empower through engagement: respond to clinical referrals with care and expertise.
• Collaborative care: work with Clinical Director to assess eligibility for hospice services.
• Direct care nursing: provide comfort and quality of life through hands-on nursing.
• Educate and advocate: empower patients and families to participate in care plans.
• Call duties support: participate in on-call coverage as needed.
