Job summary

Episcopal Homes is seeking an experienced Registered Nurse Case Manager to deliver end-of-life care and support to patients and families in Oakdale, MN. You will work independently within a supportive interdisciplinary team to manage symptom relief and quality of life.

The role requires an MN RN license, strong assessment and communication skills, and a willingness to coordinate with physicians, social workers, chaplains, and home health staff.

Qualifications

• Graduate of an accredited school of nursing.
• Current Minnesota RN license.
• Strong assessment, communication, critical thinking, and documentation skills.
• Valid driver’s license and reliable transportation.

Responsibilities

• Perform initial and ongoing nursing assessments and develop individualized plans of care.
• Provide skilled nursing care focused on comfort, symptom management, and quality of life.
• Administer medications and treatments and complete medication reconciliation.
• Serve as Case Manager for assigned patients and coordinate hospice care aspects.
• Educate and support patients, families, and caregivers throughout the hospice journey.
• Coordinate medications, equipment, supplies, and community resources.
• Collaborate with physicians, social workers, chaplains, therapists, and other team members.
• Communicate changes in patient condition and obtain provider orders as needed.
