Job summary

Chapters Health is seeking an experienced RN Case Manager in Florida to assess patient and family needs, coordinate the Plan of Care with the IDT, and provide clinical, palliative, and supportive care to keep patients in their homes as long as possible.

Responsibilities include leading home visits, educating families, coordinating care, and communicating with physicians and IDT members while maintaining compassionate, patient-centered care.

Qualifications

• Current license as RN in the state of employment.
• Minimum of one year nursing experience; hospice or hospital experience preferred.
• Active BLS for healthcare professionals from the American Heart Association or Red Cross.
• Experience with EMR/EHR systems.

Responsibilities

• Provides and manages direct care to patients and families as part of the Interdisciplinary Team (IDT).
• Coordinates the Plan of Care ensuring individualized plans reflect evolving needs.
• Educates patients and families about disease, prevention, palliative care, and safety practices.
• Supports home visits and assesses home safety, medication/nutritional compliance, and DME.
