Job summary

Big Bend Hospice is seeking an RN Case Manager in Madison, FL 32340 to coordinate and oversee the interdisciplinary plan of care for hospice patients in home or community settings. You will ensure compassionate, high-quality care, develop individualized care plans, manage symptoms, and educate patients and families.

You will collaborate with physicians and the interdisciplinary team, supervise Hospice Aides and LPNs, maintain timely documentation, and serve as the clinical advocate throughout

Qualifications

• Current Florida RN license
• Minimum of one year inpatient nursing experience or hospice/home health/long-term care experience
• Excellent communication and relationship-building skills
• Strong clinical assessment and critical thinking abilities
• Valid Florida driver's license and reliable transportation
• Compassionate, professional patient care

Responsibilities

• Coordinate and oversee the interdisciplinary plan of care for hospice patients
• Develop individualized care plans and manage symptoms
• Educate patients and families about care and options
• Collaborate with physicians and the interdisciplinary team
