Job summary

HIA Health in Detroit Lakes, MN is seeking a Hospice Staff RN/Case Manager to provide nursing care to hospice patients in coordination with the interdisciplinary team.

The full-time role, based at the Detroit Lakes office, includes admissions, assessments, symptom management and emotional support for patients and families.

Qualifications

• Minimum of RN degree; BSN preferred.
• Minimum of one year of experience as a hospital staff nurse and/or minimum of two years of experience in health care.
• Strong interpersonal communication, counseling, and teaching skills.
• Flexibility in scheduling; team player.
• Familiarity with principles related to dying and death strongly preferred.

Responsibilities

• Provide nursing care to hospice patients per IDG goals.
• Coordinate admissions, referrals, assessments, and symptom management.
• Offer emotional support to patients and families.
