Hospice of the Piedmont is seeking a dedicated Hospice RN Case Manager to deliver compassionate, high-quality care to patients and families throughout the hospice journey. You will perform initial nursing evaluations, continuously reassess patient needs, and coordinate with physicians and the health team to ensure dignified end-of-life support.

Qualifications

• Two years of relevant nursing experience is preferred.
• Graduation from an accredited nursing school and a valid RN license in Virginia.

Responsibilities

• Provide a nursing evaluation visit to new admissions.
• Regularly re-evaluate patient needs.
• Consult with the patient’s physician.
• Complete bedside documentation using the EMR.
• Coordinate and manage services offered by health team members.
• Help ensure the final chapter of your patient’s life is one of quality, compassion, and dignity.
