Job summary

Good Samaritan Hospice is seeking a compassionate Registered Nurse Case Manager to provide expert clinical care, guidance, and comfort to patients and families at end-of-life.

As part of the Interdisciplinary Team, you will perform comprehensive nursing assessments, develop and evaluate plans of care, and educate families on disease progression and symptom management, while maintaining flexible hours in the New River Valley service area.

Qualifications

• Virginia-licensed RN with 2+ years nursing experience.
• Hospice or home health experience preferred.
• Strong clinical judgment, critical thinking, and decision-making skills.
• Excellent communication and patient/family education abilities.
• Proficient with EMR documentation and computer systems.
• Valid driver’s license and reliable transportation with auto insurance.

Responsibilities

• Serve as a key member of the Interdisciplinary Team providing holistic hospice care.
• Perform comprehensive nursing assessments, admissions, and ongoing reassessments.
• Develop, implement, and evaluate individualized care plans with IDT and attending physicians.
• Provide skilled nursing care including symptom management and IV therapy.
• Educate patients and families on disease progression, medications, and caregiving tasks.
• Offer emotional support to patients and families throughout the hospice journey.
• Fulfill on-call responsibilities and assist with admissions during staffing needs.
