Good Samaritan Hospice in Lynchburg, VA is seeking a compassionate Registered Nurse to join our team as an RN Case Manager. You will provide expert case management, coordinate care for patients and families, and collaborate with the IDT to develop individualized Plans of Care.

Qualifications

• Graduate of an accredited nursing program with an active Virginia RN license.
• Two years nursing experience in hospice/home health or acute care preferred.

Responsibilities

• Provide case management and coordinate care for assigned patients and families.
• Complete comprehensive assessments, admissions, and ongoing evaluations of patient needs.
• Collaborate with the IDT to develop and implement individualized Plans of Care.
• Deliver skilled nursing care including symptom management and physical assessments.
• Educate patients and caregivers on medications, pain management, and daily care needs.
• Monitor patient condition and communicate changes to physicians and care team members.
• Maintain accurate documentation in compliance with hospice policies and regulations.
• Provide emotional support and guidance to patients and families.
• Participate in IDT meetings, on-call rotation, and quality improvement activities.
